Please be on time for your new patient appointment. Your initial visit will take about an hour with the doctor.
If for any reason you will be unable to keep your appointment, please notify the office at least 24 hours in advance so that we can accommodate other patients. The charge for a missed new patient appointment without 24 hours notice is $85.00.
Our new Electronic Health Records system provides you the convenience of completing some of your forms online. Your privacy will be carefully protected.

If you cannot complete these forms before arrival or need assistance in completing them, please come in 1/2 hour before your appointment to complete them. Failure to do so may result in a delay in your examination and treatment or the necessity to reschedule your appointment.
